Change of Address

Any time you have a change of your mailing address or phone number, please keep the Fund Office informed.  Blue Cross & Blue Shield, Delta Dental,  Mass Mutual, and the Local 441 Pension & 401(k) offices all get their information from US!  If we don't know where you are, neither will they!

You can change your address with us in several different ways.  You can:

  1. Fill out the form below, 

    • hit SUBMIT ADDRESS CHANGE and it will be emailed to the Fund Office and you're done!

    • Or, if you prefer not to electronically submit your address change, after completing the form, you can print out this page (by hitting the print icon below), and then fax it to us at 316-264-9245, or mail it to us at 505 S Broadway, Ste. 117, Wichita, KS  67202, or

  2. Call us at 316-264-2339
